अफगाजनस्ट्तान, ✔ ✔* ✔ बांग्लािेि और पादकस्ट्तान के अल्पसख्ं यक समुिायों (हहिं ,ू जसख, बौद्ध, िैन, पारसी और ईसाइयों) को िारी िैध िीर्यकाजलक िीिा (एलटीिी) 8. अन्य जििेिी नागररकों के जलए - िैध जििेिी पासपोटय ✔ X* ✔ (िैध िीिा के साथ) 9. जििेिी नागररकों (जििेिी नागररकों की जििेष श्रणे ी के ✔ ✔* ✔ जलए जिनके पास पासपोटय नहीं ह)ै को जििेिी िेत्रीय पंिीकरण कायायलय (एफआरआरओ)/जििेिी पंिीकरण कायायलय (एफआरओ) द्वारा िारी िैध पंिीकरण प्रमाणपत्र या आिासीय परजमट * “सूची III – 18 िष य और उससे अजधक आयु के व्यजि के आधार नंबर के नामांकन के जलए पहचान, पता, संबंध या िन्मजतजथ प्रमाण के रूप में प्रस्ट्तुत दकए िान े िाले िस्ट्तािेि” में िर्णयत पत े के प्रमाण (पीओए) के िस्ट्तािेि के रूप म ें भी लाग ूहोंगे। नोट: (क) उपरोि सारणीबद्ध ब्यौरे म ें सूचीबद्ध कोई भी िस्ट्तािेि केिल तभी स्ट्िीकायय होगा िब िह जनम्नजलजखत ितों को पूरा करता हो, नामत: — (i) यह ितयमान म ें िैध ह;ै (ii) जिस व्यजि के संबंध म ेंऐसा िस्ट्तािेि िारी दकया गया ह ैिह उसका हकिार ह;ै (iii) िस्ट्तािेि म ेंअंतर्ियष्ट सूचना स्रोत से सत्यापन योग्य ह;ै और4 THE GAZETTE OF INDIA : EXTRAORDINARY [PART III—SEC.4] (iv) िस्ट्तािेज़ िारी करने िाल े प्राजधकारी ने यह र्ोषणा नहीं की ह ै दक ऐसा िस्ट्तािेज़ पहचान, पते, िन्म जतजथ या ररश्त ेका प्रमाण नहीं ह ैजिसके जलए ऐसा िस्ट्तािेज़ प्रस्ट्तुत दकया गया ह।ै (ख) पहचान, पता, िन्मजतजथ और संबंध को साजययत करन े के जलए प्रस्ट्ततु िस्ट्तािेिों की स्ट्िीकाययता के मानिडं : (i) दकसी िस्ट्तािेि को पहचान प्रमाण (पीओआई) िस्ट्तािेि के रूप में तभी स्ट्िीकार दकया िाएगा यदि उसम ें व्यजि का नाम और फोटोग्राफ ह।ै (ii) नामांकन के जलए प्रस्ट्तुत दकए िाने िाल े पीओआई, पीओए, पीओआर और पीडीबी िस्ट्तािेि नामाकंन चाहन े िाले व्यजि के नाम पर िारी होना आिश्यक ह।ै (iii) दकसी िस्ट्तािेि को संबंध का प्रमाण (पीओआर) िस्ट्तािेि के रूप म ें केिल तभी स्ट्िीकार दकया िाएगा यदि उसमें पररिार के मुजखया (एचओएफ) और व्यजि का नाम ह।ै (iv) सहायक िस्ट्तािेि म ें उजल्लजखत व्यजि का नाम, व्यजि के आधार म ें उसी रूप म ें िोहराया िाएगा। नाम म ें िाजमल करने के जलए दकसी अजतररि सूचना पर जिचार नहीं दकया िाएगा। (v) पीओआई, पीओआर और पीडीबी के रूप में प्रस्ट्ततु दकए िाने िाल े सभी िस्ट्तािेिों म ें व्यजि के नाम और िन्मजतजथ म ेंसमानता होनी चाजहए। (ग) बाल िेखभाल संस्ट्थान और जििेिी नाजगरको को छोड़कर पाचं िषय से कम आयु के बालकों/बाजलकाओं के जलए पररिार का मुजखया (एचओएफ) आधाररत नामांकन अजनिायय ह।ै (घ) पररिार का मुजखया और संबंध का प्रमाण आधाररत नामांकन के जलए मानिंडः (i) एचओएफ आधाररत नामांकन करने से पूिय एचओएफ के पास िैध आधार होना आिश्यक ह।ै (ii) माता-जपता में स ेकोई एक या कानूनी संरिक, पररिार का मुजखया (एचओएफ) बन सकता ह।ै (iii) एचओएफ आधाररत नामांकन के जलए माता-जपता िोनों का आधार नंबर अपेजित ह।ै यदि बालक/बाजलका माता-जपता म ें स े केिल एक या कानूनी संरिक के साथ रह रहा ह,ै तो माता–जपता म ें से केिल एक का या कानूनी संरिक का आधार दिया िा सकता ह।ै (iv) माता-जपता में स ेदकसी एक या कानूनी संरिक द्वारा बायोमेररक प्रमाणीकरण अजनिायय ह।ै (v) एचओएफ के आधार म ेंउजल्लजखत पत े का उपयोग बालक/बाजलका के आधार में पते के जलए दकया िाएगा। (ङ) ओसीआई काडयधारकों, नपे ाल और भूटान नागररकों, एलटीिी धारकों और अन्य जििेिी नागररकों को िारी आधार की िैधता अिजध नीचे जिजनर्ियष्ट ह:ै (i) ओसीआई काडयधारक– िस साल (ii) नेपाल/भूटान नागररक – िस साल (iii) एलटीिी धारक – एलटीिी की िैधता तक (iv) अन्य जििेिी नागररक – िीिा/एफआरआरओ/एफआरओ द्वारा िारी पंिीकरण प्रमाणपत्र या आिासीय परजमट (िैसा लाग ूहो) की िैधता तक[भाग III—खण् ड 4] भारत का रािपत्र : असाधारण 5 सचू ी II - संबंध को साजययत करन े के जलए प्रस्ट्ततु िस्ट्तािेिों की स्ट्िीकाययता के मानिडं : (i) दकसी िस्ट्तािेि को पहचान प्रमाण (पीओआई) िस्ट्तािेि के रूप में तभी स्ट्िीकार दकया िाएगा यदि उसम ें व्यजि का नाम और फोटोग्राफ ह।ै (ii) नामांकन के जलए प्रस्ट्तुत दकए िाने िाल े पीओआई, पीओए, पीओआर और पीडीबी िस्ट्तािेि नामाकंन चाहन े िाले व्यजि के नाम पर िारी होना आिश्यक ह।ै (iii) दकसी िस्ट्तािेि के संबंध का प्रमाण (पीओआर) िस्ट्तािेि के रूप म ें केिल तभी स्ट्िीकार दकया िाएगा यदि उसमें पररिार के मुजखया (एचओएफ) और व्यजि का नाम ह।ै (iv) सहायक िस्ट्तािेि म ें उजल्लजखत व्यजि का नाम, व्यजि के आधार म ें उसी रूप म ें िोहराया िाएगा। नाम म ें िाजमल करने के जलए दकसी अजतररि सूचना पर जिचार नहीं दकया िाएगा। (v) पीओआई, पीओआर और पीडीबी के रूप में प्रस्ट्ततु दकए िाने िाल े सभी िस्ट्तािेिों म ें व्यजि के नाम और िन्मजतजथ म ेंसमानता होनी चाजहए। (ग) पररिार का मुजखया और संबंध का प्रमाण आधाररत नामांकन के जलए मानिंडः (i) एचओएफ आधाररत नामांकन करने से पूिय एचओएफ के पास िैध आधार होना आिश्यक ह।ै (ii) माता-जपता में स ेकोई एक या कानूनी संरिक पररिार का मुजखया (एचओएफ) बन सकता ह।ै (iii) एचओएफ आधाररत नामांकन के जलए माता-जपता िोनों का आधार नंबर अपेजित ह।ै यदि बालक/बाजलका माता-जपता म ें स े केिल एक या कानूनी संरिक के साथ रह रहा ह,ै तो माता–जपता म ें स े केिल एक का या कानूनी संरिक का आधार दिया िा सकता ह।ै (iv) माता-जपता में स ेदकसी एक या कानूनी संरिक द्वारा बायोमेररक प्रमाणीकरण अजनिायय ह।ै (v) एचओएफ के आधार म ेंउजल्लजखत पत े का उपयोग बालक/बाजलका के आधार में पते के जलए दकया िाएगा। (र्) 5 िषय और उससे अजधक तथा 18 िष य से कम आयु के व्यजियों को पररिार के मुजखया आधाररत नामांकन के माध्यम से नामांकन करने के जलए प्रोत्साजहत दकया िाता ह।ै हालांदक, यदि एचओएफ या संबंध प्रमाण िस्ट्तािेि उपलब्ध न होने पर, ऐसे व्यजि पीओआई, पीओए और पीडीबी िस्ट्तािेिों का उपयोग करके िस्ट्तािेि आधाररत नामांकन के िररए नामांकन कर सकते ह।ैं (ङ) ओसीआई काडयधारकों, नपे ाल और भूटान नागररकों, एलटीिी धारकों और अन्य जििेिी नागररकों को िारी आधार की िैधता अिजध नीचे जिजनर्ियष्ट ह:ै (i) ओसीआई काडयधारक– िस साल (ii) नेपाल/भूटान नागररक – िस साल (iii) एलटीिी करन े के जलए प्रस्ट्ततु िस्ट्तािेिों की स्ट्िीकाययता के मानिडं : (i) दकसी िस्ट्तािेि को पहचान प्रमाण (पीओआई) िस्ट्तािेि के रूप म ें तभी स्ट्िीकार दकया िाएगा यदि उसम ें उस व्यजि का नाम और फोटोग्राफ ह।ै (ii) दकसी िस्ट्तािेि को संबंध के प्रमाण (पीओआर) िस्ट्तािेि के रूप में केिल तभी स्ट्िीकार दकया िाएगा यदि उसमें पररिार के मुजखया (एचओएफ) और व्यजि का नाम ह।ै (iii) नामांकन के जलए प्रस्ट्तुत दकए िान े िाल े पीओआई, पीओए, पीओआर और पीडीबी िस्ट्तािेि नामाकंन चाहने िाले व्यजि के नाम पर िारी होना आिश्यक ह।ै (iv) सहायक िस्ट्तािेि म ें उजल्लजखत व्यजि का नाम, व्यजि के आधार म ें उसी रूप म ें िोहराया िाएगा। नाम म ें िाजमल करने के जलए दकसी अजतररि सूचना पर जिचार नहीं दकया िाएगा। (v) पीओआई, पीओआर और पीडीबी के रूप में प्रस्ट्तुत दकए िान े िाले सभी िस्ट्तािेिों में व्यजि के नाम और िन्मजतजथ म ेंसमानता होनी चाजहए। (ग) पररिार का मुजखया और संबधं का प्रमाण आधाररत अद्यतन के जलए मानिंडः (i) एचओएफ आधाररत नामांकन करने से पूिय एचओएफ के पास िैध आधार होना आिश्यक ह।ै (ii) माता-जपता में स ेकोई एक या कानूनी संरिक, पररिार का मुजखया (एचओएफ) बन सकता ह।ै (iii) एचओएफ आधाररत नामांकन के जलए माता-जपता िोनों का आधार नंबर अपेजित ह।ै यदि बालक/बाजलका माता-जपता म ें स े केिल एक या कानूनी संरिक के साथ रह रहा ह,ै तो माता–जपता म ें स े भारतीयों (एनआरआई) के जलएः - आधार नबरं धारक को उपरोि सूची म ें जिजनर्ियष्ट िन्मजतजथ के प्रमाण के रूप म ें स्ट्िीकायय िस्ट्तािेिों म ें से कोई एक िस्ट्तािेि प्रस्ट्तुत करना होगा। (ii) यदि आधार नंबर धारक न े पहले िन्मजतजथ प्रमाण के रूप में िन्म प्रमाणपत्र प्रस्ट्ततु दकया था, तो आधार नंबर धारक को उसी िन्म पंिीकरण नंबर (बीआरएन) िाला संिोजधत िन्म प्रमाणपत्र प्रस्ट्ततु करना होगा। (iii) यदि आधार नंबर धारक न े पहल े िन्म प्रमाणपत्र को छोड़कर िन्मजतजथ प्रमाण के रूप में कोई और िस्ट्तािेि (िैसे माकयिीट, पासपोटय आदि) प्रस्ट्तुत दकया था, तो आधार नंबर धारक को उसी िस्ट्तािेि का संिोजधत संस्ट्करण या िन्म प्रमाणपत्र प्रस्ट्तुत करना होगा। (iv) जििाह के पश्चात अपने नाम में पररितयन कर चुकी मजहला की िन्मजतजथ का अद्यतन करने के मामले म,ें जििाह से पूिय िारी दकए गए िन्मजतजथ प्रमाण िस्ट्तािेिों को, नाम पररितनय के सायय (उिाहरणाथ य पुरान े नाम के साथ आधार या िोनों नाम के साथ जििाह प्रमाणपत्र या कोई अन्य स्ट्िीकायय पीओआई िस्ट्तािेि) के साथ स्ट्िीकार दकया िाएगा। (v) आधार में िन्मजतजथ अद्यतन करने का अनरु ोध केिल इस संबधं में िारी मानक संचालन प्रदक्रया (एसओपी) के अनुसार ही दकया िाएगा[@]। (ङ) जिजनयम 19(3) में जनधायररत सीमा से परे आधार में नाम और हलगं अद्यतन के जलए अनरु ोध करना, ऐसी सूचना का अद्यतन केिल नीचे िी गई जिजनर्ियष्ट प्रदक्रया के अनुसार दकया िाएगाः (i) नाम – [$] (ii) हलंग – [$] (च) बालक/बाजलका (0-18 िषय) जिनके आधार में नाम "बेबी ऑफ..." ह,ै के मामले म,ें िन्म और मुत्यु रजिस्ट्रीकरण अजधजनयम, 1969 के (c) Criteria for Head of Family and Proof of Relationship based update: (i) HoF must have a valid Aadhaar before performing HoF based update. In case the child is living with only one parent or living with a legal guardian, the Aadhaar number of only such parent or legal guardian may be given. (iv) Biometric authentication by one of the parents or the legal guardian is mandatory. (v) The address mentioned in the Aadhaar of HoF will be used for the address in the Aadhaar of the individual. (d) #Request for update of date of birth in Aadhaar shall be accepted, in the following manner: (i) If date of birth is recorded as declared or approximate: a. For resident Indians below 18 years of age: - Aadhaar number holder shall mandatorily submit birth certificate as proof of date of birth. b. For Non-Resident Indian (NRI) below 18 years of age: - Aadhaar number holder shall submit birth certificate or Indian Passport as proof of date of birth. c. For resident Indians and Non-Resident Indian (NRI) of 18 years and above of age: - Aadhaar number holder shall submit any of the acceptable proof of date of birth documents as specified in the above list. (ii) If the Aadhaar number holder had earlier submitted birth certificate as proof of date of birth, the Aadhaar number holder shall submit corrected birth certificate bearing same Birth Registration Number (BRN). (iii) If the Aadhaar number holder had earlier submitted any proof of date of birth document except birth certificate (for e.g. Marksheet, Passport etc.), the Aadhaar number holder shall submit corrected version of the same document or submit a birth certificate. (iv) In case of date of birth update of woman who has changed her name post-marriage, proof of date of birth documents issued pre-marriage will be accepted along with evidence of name change (for eg. Aadhaar with old name or marriage certificate with both names or any other acceptable PoI document). (v) Request for update of date of birth in Aadhaar shall be done only in accordance with standard operating procedure issued in that regard [@]. (e) Request for update of name and gender in Aadhaar beyond the limits prescribed in Regulation 19 (3), update of such information shall be done only in accordance with the process as specified below: (i) Name – [$] (ii) Gender – [$] (f) In case of child (0-18 years) having name as “Baby of ….” in Aadhaar, the first update request for full name change shall be allowed by submission of Birth certificate issued under the Registration of Births and Deaths Act, 1969 and the rules made thereunder. (g) Head of Family (HoF) based minor updates in name (viz. addition/ deletion/ change in surname, change in spelling owing to phonetics, expansion/ abbreviation provided the primary name remains unchanged) of a minor (below 18 years of age) shall be permitted based on “Birth Certificate” or “document to prove legal guardianship” bearing the revised name. (h) Head of Family (HoF) based address update of Aadhaar may be used in case individual does not have PoA documents.
