From the Civil Surgeon, Secretariat Dispensary, Hyderabad, Letter Rc.No.2313/CDS/12, dt.19-01-2012. * * * ORDER: Sri Mohd. Yousuf Ali, Typist (Retired), Planning Department, in his application 2nd read above has stated that he has undergone Diabetes & Hypertension treatment at Government Civil Dispensary, A.P. Secretariat, Hyderabad, during the period from 02-09-2011 to 01- 01-2012, as outpatient. In that connection, he has incurred an expenditure of Rs.5,039/- for purchase of medicines and requested for reimbursement of the same. He has also enclosed Essentiality Certificate and Original Bills etc. 2) The original bills were scrutinized by the Civil Surgeon, Secretariat Dispensary, A.P. Hyderabad and recommended Rs.5,039/- as net admissible amount vide his letter 4th read above. 3) In terms of the orders issued in the G.O. 1st read above and as recommended by the Civil Surgeon, Secretariat Dispensary, A.P. Hyderabad, an amount of Rs.5,039/- (Rupees Five thousand and thirty nine only) is hereby sanctioned to Sri Md. Yousuf Ali, Typist (Retired), Planning Department towards reimbursement of the medical expenses incurred for his Diabetes & Hypertension treatment at Government Civil Dispensary, A.P. Secretariat, Hyderabad, during the period from 02-09-2011 to 01-01-2012, as outpatient. 4) The amount sanctioned in para (3) above shall be debited to “2052- Secretariat General Services – 090 – Secretariat – SH (07) – Planning Department – 010 – Salaries – 017 – Medical Reimbursement”.
