From Sri P.B. Sundar Raj, Deputy Director(Retired), Planning Dept., Dt.20-12-2011 3. Government Memo.No.9521/Plg.XX/2011-1, Dt;21-12-2011 4. From the Civil Surgeon, Secretariat Dispensary, A.P., Hyderabad, Letter Rc.No.2445/CDS/12, Dt;20-01-2012 * * * ORDER:- Sri P.B. Sundar Raj, Deputy Director(Retired), Planning Department in his application 2nd read above, has stated that he has undergone Respiratory Obstructive Airways Disorder treatment at Government Civil Dispensary, A.P. Secretariat, Hyderabad, during the period from 10-10-2011 to six months, as outpatient. In that connection, he has incurred an expenditure of Rs.8060/- for purchase of medicines and requested for reimbursement of the same. He has also enclosed Appendix-II, Essentiality Certificate and Original Bills etc. 2) The original bills were scrutinized by the Civil Surgeon, Secretariat Dispensary, Secretariat, A.P., Hyderabad and recommended Rs.8060/- as net admissible amount vide his letter 4th read above. 3) In terms of the orders issued in the G.O. 1st read above and as recommended by the Civil Surgeon, Secretariat Dispensary, A.P. Hyderabad, an amount of Rs.8,060/- (Rupees eight thousand and Sixty only) is hereby sanctioned to Sri P.B. Sundar Raj, Deputy Director(Retired), Planning Department towards reimbursement of the medical expenses incurred by him for his Respiratory Obstructive Airways Disorder treatment at Government Civil Dispensary, A.P. Secretariat, Hyderabad, during the period from 10-10-2011 to six months, as outpatient. 4) The amount sanctioned in para (3) above shall be debited to “2052- Secretariat General Services – 090 – Secretariat – SH (07) – Planning Department – 010 – Salaries – 017 – Medical Reimbursement”.
