Srinivasa Rao Assistant Section Officer, Planning Department in her application 2nd read above, has requested letter of Credit (LOC) for 1st spell Master Health Checkup in Global Hospitals for self. The LOC has been issued by this department vide reference 3rd read above. The Global Hospital, Lakdi-ka-pool, Hyderabad has furnished medical bills for Rs.3,000/- in the reference 4th read above. 2. The medical bills were scrutinized by the Civil Surgeon Specialist, Govt. Civil Dispensary, A.P. Secretariat, Hyderabad and found the net admissible amount as Rs.3,000/- vide reference 6th read above. 3. As per G.O.Ms.No:105, HM&FW (K1) Dept, dt: 09-04-2007, sanction is hereby accorded for an amount of Rs.3,000/- (Rupees Three thousand only) for payment to Global Hospital, Lakdi-ka-pool, Hyderabad, towards Medical bills for 1st spell Master Health Checkup of Sri T. Srinivasa Rao Assistant Section Officer, Planning Department, on 28-11-2011 4. The amount sanctioned in para (3) above shall be debited to “3451- Secretariat Economic Services – 090 – Secretariat – SH (07) – Planning Department – 010 – Salaries – 017 – Medical Reimbursement”. 5.
