Savithri Section Officer, Planning Department in her application 2nd read above, has stated that she has undergone 1st Spell Dental treatment at KNM Smile Dental Hospital, Dilsukhnagar, A.P., Hyderabad, during the period from 02-11-2011 to 11-11-2011 as outpatient. In that connection, she has incurred an expenditure of Rs.6,000/- for treatment and requested for reimbursement of the same. She has also enclosed Essentiality Certificate, Discharge Summary, Appendix-II and Original Bills etc. 2) The original bills were scrutinized by the Civil Surgeon, Secretariat Dispensary, Secretariat, Hyderabad and recommended Rs.6,000/- as net admissible amount vide his letter 4th read above. 3) In terms of the orders issued in the G.O. 1st read above and as recommended by the Civil Surgeon, Secretariat Dispensary, Hyderabad, an amount of Rs.6,000 (Rupees Six thousands only) is hereby sanctioned to Smt. Y.
