Seshagiri Rao, Deputy Secretary to Government (Retired)`, Planning Department, in his application 2nd read above has stated that his mother has undergone treatment DM & Cardiac Problem at Civil Dispensary, Secretariat, A.P. Hyderabad, during the period from October 2011 to December 2011, as outpatient. In that connection, he has incurred an expenditure of Rs.1,540/- for purchase of medicines and requested for reimbursement of the same. He has also enclosed Essentiality Certificate, Appendix-II and Original Bills etc. 2) The original bills were scrutinized by the Civil Surgeon Specialist, Secretariat Dispensary, A.P. Hyderabad and recommended Rs.1,540/- as net admissible amount vide his letter 4th read above. 3) In terms of the orders issued in the G.O. 1st read above and as recommended by the Civil Surgeon Specialist , Secretariat Dispensary, A.P., Hyderabad, an amount of Rs.1,540/- (Rupees One thousand five hundred and forty only) is hereby sanctioned to Sri K. Seshagiri Rao, Deputy Secretary to Government (Retired), Planning Department towards reimbursement of the medical expenses incurred by the officer for his mother, has undergone treatment DM & Cardiac Problem at Civil Dispensary, .
