From Sri K. Krishna, Section Officer, Planning Dept., Dt.20-09-2011. 3. Government Memo.No. 9538/Plg.XX/2011, Dt.23-12-2011 addressed to the Civil Surgeon, Secretariat Dispensary, Hyderabad. 4. From the Civil Surgeon Specialist, Secretariat Dispensary, Hyderabad, Letter Rc.No. 2446/CDS/12, Dt.21-01-2012. * * * ORDER: Sri K. Krishna, Section Officer, Planning Department, in his application 2nd read above has stated that his wife has undergone CAD follow-up treatment at Government Civil Dispensary, A.P. Secretariat, Hyderabad, during the period from 12-12-2011 to Six months, as outpatient. In that connection, he has incurred an expenditure of Rs.5,115/- for purchase of medicines and requested for reimbursement of the same. He has also enclosed Essentiality Certificate, Emergency Certificate and Original Bills etc. 2) The original bills were scrutinized by the Civil Surgeon Specialist, Secretariat Dispensary, A.P. Hyderabad and recommended Rs.5,115/- as net admissible amount vide his letter 4th read above. 3) In terms of the orders issued in the G.O. 1st read above and as recommended by the Civil Surgeon Specialist, Secretariat Dispensary, A.P., Hyderabad, an amount of Rs.5,115/- (Rupees Five thousand one hundred and Fifteen only) is hereby sanctioned to Sri K. Krishna, Section Officer, Planning Department towards reimbursement of the medical expenses incurred by the officer for his Wife’s CAD follow-up treatment at Government Civil Dispensary, A.P. Secretariat, Hyderabad, during the period from 12-12-2011 to Six months, as outpatient, as outpatient.
