Government Memo.No. 527/Plg.XX/2012-1, Dt.24-01-2012 addressed to the Civil Surgeon, Secretariat Dispensary, Hyderabad. 4. From the Civil Surgeon Specialist, Secretariat Dispensary, Hyderabad, Letter Rc.No. 213/CDS/12, Dt.31-01-2012. * * * ORDER: Sri A. Dayakar Reddy, Joint Secretary to Government, Planning Department, in his application 2nd read above has stated that his Mother has undergone Asthma treatment at Government Civil Dispensary, A.P., Secretariat, Hyderabad, during the period from 21-12-2011 to 21-01-2012, as outpatient. In that connection, he has incurred an expenditure of Rs.1,466/- for purchase of medicines and requested for reimbursement of the same. he has also enclosed Essentiality Certificate, Emergency Certificate and Original Bills etc. 2) The original bills were scrutinized by the Civil Surgeon Specialist, Secretariat Dispensary, A.P. Hyderabad and recommended Rs.1,466/- as net admissible amount vide his letter 4th read above. 3) In terms of the orders issued in the G.O. 1st read above and as recommended by the Civil Surgeon Specialist, Secretariat Dispensary, A.P., Hyderabad, an amount of Rs.1,466/- (Rupees One thousand four hundred and sixty six only) is hereby sanctioned to Sri A. Dayakar Reddy, Joint Secretary to Government, Planning Department towards reimbursement of the medical expenses incurred by the officer for his Mother has undergone Asthma treatment at Government Civil Dispensary, A.P., Secretariat, Hyderabad , during the period from 21-12-2011 to 21-01-2012, as outpatient.
