PLANNING (PLG.XX) DEPARTMENT G.O.Rt.No. 344 Dated.13-03-2012 Read the following:- 1. G.O.Ms.No.74, HM&FW (K.1) Department, Dt.15-03-2005. 2. From Smt.G. Surya Prabhavathi, Section Officer (Retired), Planning Dept., dt.24-01-2012. 3. Government Memo.No.581/Plg.XX/2012-1, dt.24-01-2012. 4. From the Civil Surgeon, Secretariat Dispensary, Hyderabad, Letter Rc.No.212/CDS/2012, dt.31-01-2012. * * * ORDER: Smt.G. Surya Prabhavathi, Section Officer (Retired), Planning Department, in her application 2nd read above has stated that her husband’s Right Eye Immature Cataract treatment at Srikiran Institute of Ophthalmology, Kakinada, during the period from 20-12-2011 to 21-12-2011, as inpatient. In that connection, she has incurred an expenditure of Rs.16,000/- for purchase of medicines and requested for reimbursement of the same. She has also enclosed Essentiality Certificate , Discharge summary, Emergency Certificate and Original Bills etc. 2) The original bills were scrutinized by the Civil Surgeon, Secretariat Dispensary, A.P. Hyderabad and recommended Rs.14,000/- as net admissible amount vide his letter4th read above. 3) In terms of the orders issued in the G.O. 1st read above and as recommended by the Civil Surgeon, Secretariat Dispensary, A.P. Hyderabad, an amount of Rs.14,000/- (Rupees Fourteen thousand only) is hereby sanctioned to Smt. G. Surya Prabhavathi, Section Officer (Retired), Planning Department towards reimbursement of the medical expenses incurred for her husband’s Right Eye Immature Cataract treatment at Srikiran Institute of Ophthalmology, Kakinada, during the period from 20-12-2011 to 21-12-2011, as inpatient. 4) The amount sanctioned in para (3) above shall be debited to “2052- Secretariat General Services – 090 – Secretariat – SH (07) – Planning Department – 010 – Salaries 017 – Medical Reimbursement”. 5) The Planning (XII) Department shall draw and Credit the amount to SB A/C No: 000127526697, Indian Overseas Bank, Kalyan Nagar Branch, Hyderabad (IFSC NO: IOBA0001275) of the individual.
