From the Civil Surgeon Specialist, Secretariat Dispensary, Hyderabad, Letter Rc.No.412/CDS/12, Dt.25-02-2012. * * * ORDER: Sri B. Gyaneshwar, DR&T Assistant, Planning Department, in his application 2nd read above has stated that his Mother has undergone Ischaemia CVA / Right Hemiplegia APHASIA / PD / HTN & DM treatment at Sai Krishna Super Speciality Neuro Hospital, Kachiguda, A.P., Hyderabad, during the period from 28-01-2012 to 08-02-2012, as inpatient. In that connection, he has incurred an expenditure of Rs.48,383.50 Ps for purchase of medicines and treatment and requested for reimbursement of the same. He has also enclosed Essentiality Certificate, Discharge Summary, Emergency Certificate, Appendix-II and Original Bills etc. 2) The original bills were scrutinized by the Civil Surgeon Specialist, Secretariat Dispensary, A.P. Hyderabad and recommended Rs.46,573.50 Ps as net admissible amount vide his letter 4th read above. 3) In terms of the orders issued in the G.O. 1st read above and as recommended by the Civil Surgeon Specialist, Secretariat Dispensary, A.P. Hyderabad, an amount of Rs.46,573.50 Ps (Rupees Forty six thousand five hundred seventy three and paisa fifty only) is hereby sanctioned to Sri B. Gyaneshwar, DR&T Assistant, Planning Department towards reimbursement of the medical expenses incurred by him for his Mother’s Ischaemia CVA / Right Hemiplegia APHASIA / PD / HTN & DM treatment at Sai Krishna Super Speciality Neuro Hospital, Kachiguda, A.P., Hyderabad, during the period from 28-01-2012 to 08-02-2012, as inpatient. 4) The amount sanctioned in para (3) above shall be debited to “2052- Secretariat General Services – 090 – Secretariat – SH (07) – Planning Department – 010 – Salaries – 017 – Medical Reimbursement”. 5) The Planning (XII) Department shall draw and disburse the amount to the individual.
