Secretariat, Hyderabad–Sanctioned– Orders – Issued. PLANNING (PLG.XX) DEPARTMENT G.O. Rt. No. 350 Dated:14-03--2012 Read the following:- 1. G.O. Ms. No. 74, HM & FW (K1) Department,Dt.15.03.2005. 2. From Smt. M. Chudamani, Assistant Secretary to Government (Retired), Planning Department, Application , dt.22-12-2011. 3. Govt.Lr.No.9558/Plg.XX/2011, dt.10-02-2012 addressed to the Civil Surgeon, Secretariat Dispensary, A.P., Hyderabad. 4. From the Civil Surgeon Specialist, Secretariat Dispensary, A.P., Hyderabad, Lr. Rc. No.359/CDS/12, dt.28-02-2012. * * * ORDER: Smt. M. Chudamani, Assistant Secretary to Government(Retired), Planning Department in her application 2nd read above has stated that she has undergone HTN & DM treatment at Government Civil Dispensary, A.P. Secretariat, Hyderabad, during the period from 01-12-211 to 31-03-2012. In that connection, she has incurred an expenditure of Rs.4,892/- for purchase of medicines and tests and requested for reimbursement of the same. She has also enclosed Essentiality Certificate, Appendix-II and Original Bills etc. 2. The original bills were scrutinized by the Civil Surgeon Specialist, Secretariat Dispensary, A.P., Hyderabad and recommended for an amount of Rs.4,550/- as net admissible amount vide his letter 4th read above. 3. In terms of the orders issued in the G.O.1st read above and as recommended the Civil Surgeon Specialist, Secretariat Dispensary, AP, Hyderabad, an amount of Rs.4,550/- (Rupees Four thousand five hundred and fifty only) is hereby sanctioned to Smt. M. Chudamani, Assistant Secretary to Government(Retired), Planning Department towards medical expenses incurred by her for her HTN & DM treatment as outpatient, at Government Civil Dispensary, A.P. Secretariat, Hyderabad, during the period from 01- 12-211 to 31-03-2012 as outpatient. 3) The amount sanctioned in para (3) above shall be debited to “2052- Secretariat General Services – 090 – Secretariat – SH (07) – Planning Department – 010 – Salaries – 017 – Medical Reimbursement”. 4) The Planning (XII) Department shall draw and credit the amount to SB A/c.
