Smt K.Srilakshmi, SO, I&I Dept., Application Dt.07-09-2011. 2. Govt. Letter No.738/I&I/OP.I/2011, dated 21-09-2011 to M/s. Global Hospitals, Hyderabad. 3. From the Global Hospitals, Hyderabad letter dated 29-09-2011. 4. Govt. Memo. No.738/I&I/OP.I/2011, dated 23-01-2012 to Chief Medical Officer, AP Secretariat, Hyderabad. 5. From the Civil Surgeon Specialist, Govt. Civil Dispensary, AP Secretariat, Hyd., Lr.Rc.No.204/CDS/2012, Dated 31-01-2012 >>>>><<<< ORDER: In the reference first read above, Smt K.Srilakshmi, Section Officer, Infrastructure & Investment Department has requested Letter of Credit (LOC) for Master Health Checkup in Global Hospitals for self. The Letter of Credit has been issued by this department vide reference second read above. The Global Hospitals, Hyderabad has furnished medical bills for Rs.3,000/- in the reference 3rd read above. 2. The medical bills were scrutinized by the Civil Surgeon Specialist, Govt. Civil Dispensary, AP Secretariat, Hyderabad vide their Lr.Rc.No.204/CDS/2012, Dated 31-01-2012 and found the net admissible amount as Rs.3,000/- (Rupees Three thousand only). 3. Sanction is therefore, accorded for an amount of Rs.3,000/- (Rupees Three thousand only) for payment to “Global Hospitals, Hyderabad” towards Medical bills for Master Health check up of Smt K.Srilakshmi, Section Officer, Infrastructure & Investment Department. 4. The amount sanctioned in para (1) above shall be debited to “3451 – Secretariat Economic Services – 090 – Secretariat – SH (28) – Infrastructure and Investment Department – 010 – Salaries – 017 – Medical Reimbursement”. 5. The Drawing Officer, Infrastructure & Investment (O.P-Claims) Department shall draw the cheque in favour of ‘GLOBAL HOSPITALS, Hyderabad” for settlement of the bill. 6.
