From M/s Jyothi Computer Services, Hyderabad, Bill No.JCS/PLG/2012/03/1, dated: 21-03-2012. * * * * * O R D E R: Sanction is hereby accorded for payment of Rs. 54,381/- (Rupees Fifty four thousand three hundred and eighty one only) i.e Rs.40,293/- (Remuneration) +Rs 5,484/- (EPF @ 13.61%) + Rs. 1,914/- (ESI @ 4.75%) + Rs. 1,612/- (Agency commission @ 4%) + Rs.5,078/- (Service Tax payable on total bill @ 10.30%) to M/s Jyothi Computer Services, 3-4-496/A, 1st Floor, Jyothi Arcade, Near P.F.Office, Barkatpura, Hyderabad for having arranged the services of (5) Data Entry Operators cum Assistants for the period from 21-02-2012 to 20-03-2012. Certified that the said personnel have attended the duty during the period stated above. 2. The Asst. Secretary to Government, Planning (Claims) Department shall draw in favour of M/S Jyothi Computer Services, Hyderabad for the amount sanctioned in para (1) above, in AXIS Bank Ltd., Corporate Banking, Greenlands Branch, Hyderabad , Cash Credit A/C NO. 911030062648747, MICR Code - 500211046 and IFSC Code – UTIB0001634. 3. The Agency shall submit a certificate regarding proper remittance the statutory deductions for the present month along with the bills for the next month. 4. The expenditure sanctioned in para 1 above shall be debited to the Head of Account: M.H. 2052 – Secretariat General Services – M.H. 090 – Secretariat – S.H. (07) Planning Department – 300 Other Contractual Services’. 5. This order does not require the concurrence of Finance Department as per the orders on the subject. 6.
