ORDER :
In the reference third read above, the Commissioner of Social Welfare, A.P.,
Hyderabad has stated that her son Master B. Aditya has undergone some tests and
taken treatment for “# 2nd Metacarpal fracture” at Apollo Hospital, Hyderabad as an
out patient on 24-09-2011, for which she incurred an amount of Rs. 3,942/- and
requested to reimburse the said amount.
2. In the reference fifth read above, the Civil Surgeon Specialist, Govt. Civil
Dispensary, A.P. Secretariat, Hyderabad, has scrutinized the bills and furnished the
eligibility for an amount of Rs.3,890/-.
3. Sanction is hereby accord for reimbursement of an amount of Rs.3,890/-
(Rupees Three thousand eight hundred and ninety only) to Smt B.Udayalaxmi,
IAS., Commissioner of Social Welfare, A.P., Hyderabad towards medical expenses
incurred by her for her son Master B. Aditya has undergone some tests and taken
treatment for “# 2nd Metacarpal fracture” at Apollo Hospital, Hyderabad as an out
patient on 24-09-2011.
4. The said expenditure shall be debited to the Head of Account 2225 – Welfare
of SCs STs & OBCs – 01 – Welfare of SCs – 001 – Direction & Administration – 01 –
Head quarters officer – 010 – Salaries – 017 – Medical Reimbursement.
5. The Commissioner of Social Welfare, A.P., Hyderabad shall draw and disburse
the amount in favour of Smt B.Udayalaxmi, IAS., Commissioner of Social Welfare,
A.P., Hyderabad.
