ORDER:-
In pursuance of the orders issued in the reference 1st to 9th read above, sanction is
hereby accorded for payment of an amount of Rs. 23,963/- (Rupees Twenty Three thousand
Six Hundred and Sixty Three only) towards contribution of savings fund portion is Rs.9,396/-
(Rupees Nine thousand and Three Hundred and Ninety Six only) and towards interest thereon a
sum of Rs 14,297/- (Rupees Fourteen thousand and Two Hundred and Ninety Seven only)
under Group Insurance Scheme to Sri. N. Balakondaiah, Section Officer Panchayat Raj and
Rural Development Department, retired from service on 31.12.2011 on attaining the age of
superannuation and who was a member of the A.P. State Employees Group Insurance Scheme
with a subscription of Rs. 10/- (Rupees Ten only) per month from 11/1986 to 10/1994 @
Rs.30/- (Rupees Thirty only) per month from 11/1994 to 10/1996, Rs. 60/- (Rupees Sixty only)
per month 11/1996 to 10/2010 and Rs. 120/- (Rupees One Hundred and Twenty only) per
month from 11/2010 to 12/2011. Calculation of savings fund and interest thereon as per orders,
from time to time is as shown in Annexure appended to this order.

Government.
4. Certified that Sri. N. Balakondaiah, Section Officer (Retired), Panchayat Raj and Rural
Development Department has not availed E.O.L. Necessary entries have been made in the
Service Register of the individual.
5. The Panchayat Raj and Rural Development (OP.III) Department are requested to draw
and disburse the amount sanctioned in para 1 above to Sri. N. Balakondaiah, Section Officer
6. The individual is informed that if any excess amount is paid to her erroneously, the same
will be recovered from her with out any notice.
