ORDER:-
Sri K Rajeshwar Reddy, Assistant Section Officer (2), Home (PSC) Department, has
submitted the medical bills for reimbursement of expenses incurred by him towards his
Daughter’s treatment for TERM / AGA / EXAGERATED NNJ at Kamineni Hospitals,
L.B. Nagar, Hyderabad on 08-12-2011, for an amount of Rs. 12,657/-In the reference 2nd
read above, the medical bills have been forwarded to the Civil Surgeon Specialist
Government Civil Dispensary A.P. Secretariat. of Andhra Pradesh for scrutiny and report.
2. The Civil Surgeon Specialist Government Civil Dispensary A.P. Secretariat. of
Andhra Pradesh in his letter Lr. Rc. No. 196/CDS/2012, dated 31-01-2012, has issued
admissibility certificate for an amount of Rs. 12,177/-, after scrutiny, vide reference 3rd
read above .
3. Government hereby accord sanction for an amount of Rs. 12,177/- (Rupees Twelve
Thousand One Hundred and Seventy Seven Only) to Sri K Rajeshwar Reddy, Assistant
Section Officer (2), Home (PSC) Department, towards his Daughter’s treatment for TERM
/ AGA / EXAGERATED NNJ at Kamineni Hospitals, L.B. Nagar, Hyderabad on 08-12-
2011.
4. The expenditure sanctioned in para 3 above, shall be debited to ‘2502- Secretariat
General Services – M.H. 090 – Secretariat – S.H. (08) – Home Department – 010 salaries –
017 – Medical Reimbursement”.
5. The Home (OP-II- claims) Department are requested to draw and disburse the
amount sanctioned in para 3 above to Sri K Rajeshwar Reddy, Assistant Section Officer
(2), Home (PSC) Department.
